We will need the following information for Santa Claus prior to him making his call from the North Pole:

1. Child's Name & Age

2. Current School Teacher & Grade

3. Favorite Toy from last year's Christmas

4. Destination for Christmas (Where you will spend it this year- the address... We have seen that some children get worried Santa will not know where they are if they are out of town for Christmas- it also helps with the more personal touch)

5. Anything additional you would like to be included in the call (pet's name, favorite sport, etc)

Please fill out the information below and put any additonal information in the comment section, so we can have Santa make the call. The deadline is December 15, 2009 to make sure Santa can get each call done before he leaves the North Pole to deliver all the gifts prior to Christmas!
